Innovation Program Form

Any employee who has a suggestion related to methods, procedures, quality, empowerment, self direction or quality of work life is encouraged to fill out this form and place it in the appropriate suggestion box or campus mail. The suggestion form must include your name for you to be eligible to receive an award. Criteria for the program are outlined in the Employee Award Programs booklet.
